Silent Hill: Townfall’s “Many Into One” mission sends you through a hospital filled with monsters and locked doors, and the hardest part isn’t the fear — it’s the blood transfusion puzzle. To open the final door, you need to find six missing blood bags, put them in the right slots, and match letters to names on a circle. Here’s how to solve it.
The Locked Door and the Map
Once you complete the patient puzzle during the “Many Into One” mission, a locked door appears ahead of you. On either side of the path leading to it sit bags attached to the walls, and a machine draws blood from both of them. Three empty slots must be filled before the door will open, and doing so reveals the next stage.
The map must be taken before the machine can be reached. It sits upon the counter in front of the door. That map points to the blood transfusion machine itself and marks the rooms where the missing bags lie.
Finding Alan Duncan’s Bag
Start by taking the eastern corridor and entering the last room to the right. Crawl through the vent in the southwest corner to find Alan Duncan’s blood bag. Don’t forget to grab Zoe’s patient list and play the recording before leaving.
Hanging Alan Duncan’s bag on the door in the west corridor gives you access to the west wing.
Sarah Watson and Lesley Greer
Zoe’s patient list marks the next two bags on your map. Sarah Watson’s bag is in room 05, southwest. Lesley Greer’s bag is in room 12, northeast — but room 12 is locked for now.
Instead of facing it directly, go through rooms 3 and 4, which lets you avoid the monster entirely. Take Sarah Watson’s bag in room 5, then head down the staircase in the west. On the lower level, sneak past the monsters into room 4 to collect the Ward W Master Key. You can’ along with you, since she’s the one who can kill these monsters, and she’s also the one who will kill you instantly if they manage to grab you.
Unlock room 13 with the key, then climb up using the ladder. Go into room 12, where you will find Lesley Greer’s blood bag to take.
Matthew Clark’s Bag
Return to the area with the blood transfusion machine and use Lesley Greer’s bag to enter the east wing. Distract the monster to sneak into room 3, then enter room 2. Matthew Clark’s blood bag is attached to the corpse in the elevator — but it drops to the lower floor before you can reach it.
Don’t go downstairs yet. Crawl through the vent between the staircase and room 6 instead. Drop down into room 10, where you’ll find George McEwan’s blood bag and the Ward P Master Key. Use the key to open room 3 on the same floor level and collect Matthew Clark’s bag.
The Vent Escape
You now possess every single blood bag available. Since the nearest exits remain sealed shut, your best path lies through the vent opening in room 2, which leads to an escape route. Once upstairs, turning the wheels in rooms 5 and 10 will unlock the door back to the transfusion puzzle area. Just be cautious of the monster that cannot be killed.
Matching Bags to Slots
The machine opens when each bag’s description lines up with the patient info sitting at its slot. Put the bags in these spots:
- Sarah Watson’s blood bag
- Matthew Clark’s blood bag
- George McEwan’s blood bag
The Circle Puzzle
After the first door opens, you’ll find a second transfusion door puzzle. This time, you must connect the center of the circle to three of the eight blood bags. The name at the center is Ordell, which is Simon’s last name — Silent Hill: Townfall’s protagonist.
First, open your inventory and find Simon’s medical wristband. It shows that his blood type is A+. Then examine the plaques on the walls, which detail which blood types are compatible for donation. You will see that blood type A+ can receive from O-, O+, A-, and A+.
Study the blood bags hanging on the wall. Thomas Fergusson, Hannah McGregor, and Sarah Watson share Simon Ordell’s blood type. The answer lies in linking “Ordell” with the initials T.F, H.M, and S.W.
Finishing the Puzzle
Finish the blood transfusion puzzle by interacting with the object at the center. Then take the route to hotel room 204, which ends “Many Into One” and begins the “Begin Again” mission.

The challenge here is a pleasingly tangled means of gaining entry. One must locate six blood bags amid a network of passages, ducts, secured portals, and tokens, culminating in matching characters with names upon a ring.
